The MHSAA's requirements for Sports Physicals has been modified for the 2020-21 school year.  Student athletes will not have to obtain a new Sports Physical from a Physician as long as their health has not significantly changed. If student athletes can answer "no" to all the questions on the new MHSAA Questionnaire, then only that new form needs to be filled out and turned into the Athletic Office.

New student athletes or students who did not turn in a sports physical last year (2019-20) will need the full MHSAA Sports Physical for the 2020-21 school year (completed by a Physician) turned in to the Athletic Office.

These forms, once turned in, will be uploaded to the FinalForms Registration System that ALL Student Athletes need to fill out in order to tryout or participate in athletics at TC Central HS.
